We provide some services which are outside our NHS work and therefore attract charges based on the British Medical Association's (BMA) scale. This includes:
- Insurance claim forms
- Private sick notes issued before 7 days
- Medicals and reports for HGV, PSV and Taxi licences
- Firearm / Shotgun licences
Please contact the secretary to arrange medical examinations; she will advise you of our fees at the time of booking your appointment.
Taxi / HGV and driving medicals: Please arrange a sight test with an optician and ask the optician to complete that section of the form before booking your medical at the surgery.
Passport Forms - Due to revised regulations introduced by the Passport Office, GPs are no longer able to countersign passports unless they have known the applicant personally for at least two years (for example as a friend, neighbour or colleague and should not be someone who just knows the applicant professionally).
