Self-referral for musculoskeletal (MSK) physiotherapists

Musculoskeletal (MSK) physiotherapists assess and treat problems with muscle, bone and joints that may cause pain, stiffness and/or disability.

The team provide specialist assessment followed by expert advice and treatment for a wide range of service for any joint and muscular related problems including rehabilitation after orthopaedic surgery, sports injury rehabilitation and women’s health physiotherapy advice and treatment.

All eligible patients can now directly access NHS physiotherapy services, thorough our new self-referral service without needing to go via their GP.

Patients under the age of 16 years will need a clinical referral.

Self-referral for age related hearing loss assessment

Age Related Hearing Loss service

Oxfordshire patients can now self refer for age related hearing loss assessment.

Accessing the service

Patients who wish to self refer can use the self referral form (see link below) and contact the Provider of their choice using the details provided within the form.

Patients should self manage wax for a minimum of 2 weeks ahead of their hearing assessment.

When you and your GP agree that you need to see a specialist you will be provided with information on at least three hospitals or clinics to choose from. You will also be able to choose the date and time of your appointment. This is known as NHS E-Referral. If you have any queries concerning this service please phone the secretary on 01235 522379 between 09:30 and 13:00pm.

It is important that if at a later date you cannot make your outpatient appointment you phone the hospital to rearrange it. The Oxford Hospitals are moving to a paperless appointment reminder system. More information can be found by following the link: Hospital Appointment Reminders.

NHS e-Referral Tracking

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

To track your e-Referral you will need the following details to log into the e-Referral website:

  • Your date of birth
  • Reference number – this will be on the first page of your letter
  • Password – this will be on the second page of your letter

Travelling to Hospital for your Appointment

Public Transport

There are regular bus services to all three hospital sites; John Radcliffe and Churchill Hospitals, Headington and the Horton Hospital, Banbury.  These services are operated by Oxford Bus Company 01865 785 400 or www.oxfordbus.co.uk and Stagecoach 01865 772 250 or www.stagecoachbus.com Each hospital site has limited Pay & Display parking areas but there are links to Park & Ride services.

Non Emergency Ambulance Transport

Non emergency ambulance transport is a service that includes ambulances, ambulance & voluntary cars and taxis which are made available to get you to and from all Oxford Radcliffe Hospitals.  You will only be able to use the service if you (the patient) fall into one of the following categories:

  • Patients who require continuous oxygen
  • Patients who are unable to stand or walk more than a few steps and who cannot use public transport
  • Patients who require a stretcher
  • Patients with a disability or a genuine reason as to why they are unable to travel by private or public transport.
